Unique Media Permission Form

Dear Parent/Guardian,

We ask for Permission to Publish during the school enrollment process, but for unique media opportunities, we want to keep you informed and allow you a chance to review these permissions.

WMS Informed Consent for Release of Photos/Video (Unique Media)

Winnipeg Mennonite School is seeking your permission to share photos/videos externally of your student(s) related to this unique media opportunity. The photos/videos may be used in the following ways (this list is not exhaustive):

WMS social media accounts.
Advertising such as posters, brochures, or online ads.
WMS website and emails.
News media covering school activities.

With all photos/videos taken by WMS or approved third-party, we will ensure:

All photos/videos positively represent the students in a God-honouring way.
Requests to remove media will be honoured.
A child’s name will not be published unless we have made a specific request in our communications with you.
No photos/videos will be sold to a third-party, nor shared for profit.

I have received details of the media opportunity from a WMS staff member and understand why I am being asked to sign this release form.

We have read the WMS Informed Consent for Release of Photos/Video (Unique Media) and we understand that in allowing our child to appear in external media, we are assuming the risks associated with doing so. This consent is in effect for the duration of the unique media opportunity or until we inform WMS, in writing, of our choice to discontinue the standing consent (whichever comes first). We agree to inform our child(ren) of our wishes with regards to photos/videos.
